Don't know where you get this Eq A-2 and Eq A-3 in chapter B-XXX from.
If you want them to be Eq B-2 and Eq B-3, I suggest, you read how to
create this kind of numbering format.

Then I want to have equations like:

.. liked to this paragraph
And also

..linked only to this paragraph
...
At the end. I want to have...

Why do you want justification and tab leaders in an index? You wouldn't
have these problems if you used the common index formatting you'll find
in most published books. That formatting looks more like:
Amount A-7, A-34, A-190

Bite the bullet and create a new para tag for appendix headers if you
want properly automated formatting in your generated TOC.
